It is difficult to say with any certainty because people are lying, but it is beginning to appear that Musk approached Trump with the idea and got some sort of 'agreement' to do it. It is not clear who then 'ordered' the cutoff and whether it came from the White House via the DOD or Musk just told SpaceX to do it.

From The Telegraph story: A senior source told the Telegraph that after the chaos and violence that followed the restriction, Musk was concerned about negative consumer reactions in Europe. "He tweeted out that he was innocent and that it was the U.S. administration that did it," the source reportedly said. "The reality is that they were both in it."
