Where do I find, or how do I easily create, an empty PDF music sheet in A5 format? There are lots of alternatives out there on the internet but all of them are letter or A4 size.

I have used GNU Lilypond for this in the past (though that was about 20 years ago). It is designed as a program for typesetting music, but you can configure page sizes and have it give you a mostly empty score (just the clef[s], maybe a key signature) and nothing else.
@david_chisnall I used PMX and MusicTeX ages ago and I could most likely do something similar with that. But it takes time reviving 20 year old programming skills...
